Live at Home for Longer
As a result of the Royal Commission into Aged Care Quality and Safety, the Support at Home program, will replace the existing Home Care Package (HCP) program starting 1 July 2025.
Support at home is a simpler system for you to live safely and comfortably at home, this program prioritises early intervention, and access to higher care levels for those with complex needs.

Higher Funding Levels: With 8 classifications and 2 short term classifications
Care Partners: Dedicated care management for better outcomes
Affordable Pricing: Capped fees with no additional administrative or management costs
Equitable Contributions: Client contributions apply only to non-clinical services
Reduced Wait Times: Faster access to services, targeting a 3-month wait by 2027
Preventative Support: assistive technology and home modifications
Restorative Care Expansion: Extended support from 8 to 12 weeks
End-of-Life Pathway: compassionate care in the final months
How to Access Support at Home program
Existing Clients
If you have a Home Care Package (HCP) on 30 June 2025, your funding level and any unspent funds will transition to Support at Home.
Clients on the National Priority System and approved for a package by 30 June 2025 will receive a Support at Home budget equivalent to their package level when it becomes available.
New Clients
To access Support at Home, you need an assessment through My Aged Care.
Eligibility includes:
- People aged 65+
- Aboriginal and Torres Strait Islander
- people aged 50+
- People aged 50+ at risk of or experiencing homelessness.
The Single Assessment Tool (SAT): Simplifying Aged Care Access
From 9 December 2024, aged care assessments will transition to the Single Assessment Tool (SAT) as part of the new Single Assessment System (SAS).
The SAT replaces current assessment processes, offering a streamlined and comprehensive approach to determine eligibility, care needs, and funding. This tool creates personalised care plans tailored to individual goals, making access to aged care services simpler and more effective.
Under the new Support at Home program, a detailed service list will clarify available options, including:
Clinical care: Nursing and physiotherapy
Independence services: Personal care, respite, transport, and social support
Everyday living support: Cleaning, gardening, and meal delivery
This change demonstrates a commitment to improving assessment experiences and delivering accessible, individualised care for Australians.
